The sequel sends Juliet back to what was once San Romero, but the city has changed dramatically.
It has been rebuilt into Jaredwood, a sprawling metropolis shaped around the film industry. According to the official premise, a mysterious director traps Juliet inside his own twisted zombie movie.
Juliet must fight with her allies, uncover the conspiracy behind the chaos and stop a new threat known as the Corrupted Realm zombies.
The film-industry setting gives the series a new framework for its familiar mix of horror, black comedy and exaggerated pop imagery. Instead of simply returning to the old high school, Back2Back can build locations around a movie world that has escaped normal boundaries.
Nick also appears again in a form longtime players will immediately recognize: still accompanying Juliet as a severed head.
Combat Still Combines Cheer Attacks and Chainsaw Action
The TGS demo keeps recognizable foundations from the first game.
Juliet can perform a quick cheer attack suited to close-range openings, while chainsaw attacks are slower but cover a larger area.
The chainsaw also has a gauge that can be charged by revving it. That power ties into a chainsaw dash, expanding Juliet's mobility while keeping her signature weapon central to the movement system.
Candy remains part of the recovery system, with the TGS build allowing Juliet to restore health by using it.

At the TGS stage, the team explained that Joe Teng proposed a sequel after LOLLIPOP CHAINSAW RePOP was announced in 2022. Roughly a year of discussion followed before the project moved forward.
Development is now being handled by JP UNIVERSE, the company led by Hajime Tabata. Yasuaki Nasu serves as development director, while staff with direct knowledge of the original game have also returned.
That includes Masahiro Yuki, who worked on the original scenario, and NekoshowguN, whose character design and artwork helped define the earlier game.
The combination is intended to preserve the recognizable identity of Lollipop Chainsaw without turning Back2Back into another remaster.
“99%” Does Not Mean the Game Is Almost Finished
One piece of information circulating after the stage requires a specific correction.
An on-site Japanese interpretation made a statement sound as if LOLLIPOP CHAINSAW 2 Back2Back was 99% complete.
The official English account later explained that the stage translation was incorrect because of the audio conditions.
The intended statement was “99% of the creation we cannot tell right now.” In other words, the team was saying there is still a great deal it cannot reveal.
The number was not a development-progress percentage.
The only confirmed launch timing remains 2027, with no specific month or date announced.
TGS 2026 Is the First Chance to Play the Sequel
Back2Back is available as a playable demo at the SEGA/ATLUS booth during Tokyo Game Show 2026.
The action demo introduces Juliet's basic attacks and places her against groups of zombies. 4Gamer reports that access to the demo is restricted to attendees aged 18 or older because of its violent content, though that event restriction should not be treated as the game's final age rating.
